# Ferritin 60: is that a good level?

## Summary

For a healthy adult, a ferritin of 60 µg/L is a good result. NHS guidance regards levels above 50 µg/L as excluding iron deficiency in the absence of inflammation, and a reserve at this level absorbs the normal demands of training, donation and illness. The exception described in WHO guidance is testing during infection or inflammatory illness, when ferritin rises regardless of iron status and genuine deficiency can be hidden at results up to 70. A result of 60 taken while well means iron is very unlikely to be the issue. The same result taken while inflamed, with symptoms present, is worth repeating with a CRP once recovered. There is no supplementation case at this level.

## How it works

A ferritin of 60 µg/L indicates a working reserve, enough to buffer months of ordinary variation, recovery after blood donation, a training block, or a period of reduced appetite. Iron-dependent processes are supplied at this level, and research on iron status indicates that additional iron adds nothing in a replete person, while excess iron creates oxidative load the body has to manage. The interpretive caveat is ferritin's second role as an acute-phase protein, meaning a protein that rises during illness and inflammation. This can present a genuinely thinner iron state behind a healthy-looking number, which is why the circumstances of the test matter.

## Timing

No scheduled repeat test is needed from a result of 60 in a healthy adult, and the next routine blood panel suffices. An earlier repeat is worthwhile only where the original test was taken during illness or inflammation, in which case a repeat with CRP once recovered gives a trustworthy reading.

## Safety profile

No iron action is the appropriate action at this level. Studies of iron regulation show that supplementing at replete levels serves no purpose and works against the body's own absorption control over time. The standard cautions are unchanged. Bleeding signs or unexplained weight change belong with a GP regardless of a reassuring ferritin result.

## Special populations

In menstruating women, a result of 60 is a strong position worth maintaining. In athletes, this band covers most comfort ranges used in sports research, and performance problems at 60 are unlikely to be iron-related. In anyone tested during or shortly after illness, or with a known inflammatory condition, the WHO caveat is the main point, and a repeat test when well, ideally with CRP, comes before trusting the number. Regular blood donors at 60 have room to continue donating with dietary attention between sessions.

## Guideline positions

NHS guidance regards ferritin above 50 µg/L as excluding iron deficiency in the absence of inflammation, and the 46 to 70 band sits at or beyond that line. WHO 2020 guidance adds the conditional threshold: in people with infection or inflammation, deficiency may still exist below 70 µg/L. Guidance for specific chronic inflammatory conditions goes further, with heart failure, kidney disease and inflammatory bowel disease pathways using a ferritin of 100 alongside transferrin saturation. For the healthy majority, a result of 60 closes the iron question.

## Practical framework

The evidence-supported action at this level is maintenance rather than supplementation. A maintenance diet is unremarkable and effective. It means varied iron sources through the week, vitamin C alongside plant-based meals, and tea or coffee kept away from the main iron-containing meals. Where symptoms prompted the test and 60 was the answer, the useful conclusion is that iron is very unlikely to be the cause.
