# Folate blood test results explained: what does your number mean?

## Summary

Reading a UK folate result uses three zones. Below roughly 7 nmol/L, about 3 µg/L, is the deficiency zone. From 7 to 13 nmol/L reads as low rather than deficient. Above 13 is adequate, and no UK guideline rewards chasing higher numbers outside preconception and pregnancy, where the standing NHS advice is 400 µg folic acid daily regardless of blood results. The laboratory's units need checking before anything is read. One rule outranks the thresholds. B12 is checked before folate is corrected, because folate supplementation over an unrecognised B12 deficiency can improve the blood picture while nerve damage from the B12 problem continues.

## How it works

Folate runs the one-carbon chemistry behind DNA synthesis, which makes fast-dividing tissues, with blood first among them, the earliest casualties of shortage. The result is the same megaloblastic anaemia that B12 deficiency produces, which is exactly why the two are read together and corrected in the right order. Serum folate tracks days of intake because circulating folate turns over quickly, and red cell folate, where measured, averages the previous three to four months, on the same logic as the two iron measures on the ferritin shelf.

## Timing

Serum folate moves in days to weeks with intake, so a fortnight of genuine dietary change is visible on a retest and a month is conclusive, which makes it the fastest-responding of these blood scales. Red cell folate, where used, lags months by design. The pregnancy advice runs on the calendar, from preconception to twelve weeks, rather than on retesting.

## Safety profile

The masking rule is the one serious hazard on this scale and bears restating. Folate correction can normalise the blood count of a combined deficiency while the unrecognised B12 problem continues damaging nerves, which is why B12 status comes first and why symptoms such as pins and needles alongside any low folate result are a prompt GP matter. Folate itself is water-soluble with a generous safety margin at supplemental doses, and the care sits in the sequencing rather than in the substance.

## Special populations

Anyone planning pregnancy or in the first twelve weeks takes the NHS 400 µg folic acid regardless of this scale, with higher prescribed doses for specific risk groups their GP or midwife will know. Heavy drinkers combine poor intake with impaired handling and occupy the deficiency zone disproportionately. Coeliac and Crohn's readers should fold low results into their absorption picture with their clinical team. A small number of prescriptions interact with folate handling, which is a GP conversation.

UK laboratory practice puts the serum folate deficiency bound around 3 to 4 µg/L, roughly 7 to 9 nmol/L, and UK guidance attaches no benefit to pushing beyond the adequate range.
