An HbA1c of 48 to 53 mmol/mol meets the UK diagnostic threshold for type 2 diabetes once confirmed on a second test, or on one test with symptoms. Per NICE NG28, the first steps are structured education, diet and weight management, with metformin the usual initial prescription and a target of 48 for adults managed by healthy living or a prescription without hypoglycaemia risk. The DiRECT trial showed remission at one year in 46 percent of people on a GP-led weight programme, rising with every kilogram lost, and a start this close to 48 is the best position to be in. At 48 the pancreas has stopped fully compensating for insulin resistance and average glucose has crossed into the range where the small vessels of the eye, kidney and nerves begin to take damage over years, which is why the threshold sits here. The underlying process, fat inside the liver and pancreas suppressing insulin response and release, is the same as at 44, just further along, and DiRECT showed that reducing that fat restores pancreas function in a majority within months when the disease is recent. Metformin works on the liver's glucose output and does not cause low blood sugar, which is why it is the first prescription and why the 48 target applies on it.
Confirmation test within weeks. First re-test at three months, then every three to six months until stable. Remission in DiRECT was measured at twelve months. Eye screening annually from diagnosis.
Thirst, frequent urination, weight loss, blurred vision, vomiting or abdominal pain at diagnosis need same-day advice; very high readings can be type 1 at any age. Never add a glucose-lowering supplement to metformin or any other prescription without the GP, because of hypoglycaemia and interaction risk.
