# HDL below 1: what a low HDL means and what raises it

## Summary

HDL below 1 mmol/L in men or 1.2 in women is under the BHF guide level. On its own it is a marker rather than a target, because drugs that raised HDL without changing anything else did not cut heart attacks, so the number is read as a sign of the metabolic pattern, meaning low HDL with high triglycerides, central weight and a rising HbA1c. What raises HDL is what addresses that pattern, meaning regular aerobic activity, weight loss where there is weight to lose, stopping smoking, the saturated-to-unsaturated fat swap, and alcohol kept moderate rather than used to lift the number. Niacin and HDL-raising supplements are not recommended. HDL is read with non-HDL and QRISK3, which decide what matters.

## How it works

HDL particles collect cholesterol from artery walls and carry it to the liver, and their level falls when triglycerides are high, because the two are exchanged in the blood. A high triglyceride means HDL particles are loaded with fat, cleared faster, and fall. That is why low HDL and high triglycerides arrive together and why insulin resistance, which drives the triglyceride, drives the HDL down. Smoking damages HDL function directly, aerobic activity raises both the level and the function, and weight loss reduces the triglyceride exchange. Raising the HDL number pharmacologically did not help in trials because the particles produced were not doing the job, while raising it through the metabolic levers does, because the whole pattern improves.

## Timing

Activity raises HDL over two to three months. Smoking cessation effects build over months. Weight effects track the weight. Re-test at three months.

## Safety profile

Niacin at HDL-raising doses caused flushing and liver effects and did not reduce events in trials; it is not used for this. Alcohol is not a tool to raise HDL. ## Special populations

People with diabetes and central weight carry the low-HDL pattern most often and the glucose levers apply first. Women have higher HDL on average, which is why the guide level differs. NICE NG238 does not set an HDL target and makes statin decisions on QRISK3 and non-HDL; the HDL contributes to QRISK3 through the total-to-HDL ratio. Smoking stops, which restores HDL function within months. Weight comes down where there is weight to lose. ## Common misconceptions
