# How long does magnesium take to work?

## Summary

Magnesium works on different timelines for different things. Bowel effects from citrate or oxide arrive within 12 to 24 hours (NIH ODS). Sleep changes gradually: most measurable change by weeks two to four and a fair verdict at four to eight weeks, with sleep onset around 17 minutes faster than placebo in the pooled trials (Mah and Pitre 2021). Anxiety and stress take two to eight weeks if magnesium helps at all, on low-quality evidence (Boyle 2017). Blood pressure starts to move at four weeks and peaks by twelve (Zhang 2016). Cramps are the exception: the 2020 Cochrane review found no meaningful effect in older adults (Garrison 2020). These timelines apply where intake or level was low.

## How it works

**Why the timeline is weeks, not days.** Serum magnesium is held within a narrow range, so a supplement does not produce a measurable rise in most people; tissue and intracellular stores refill over weeks. The effects on sleep, mood and blood pressure act through calcium channel modulation, GABA-A receptor support and NMDA receptor antagonism, which follow tissue repletion rather than a single dose (Pickering 2020). The bowel effect is different in kind: it is osmotic and needs no repletion.

## Effective dose

**How much, and when to judge it.** The trials that showed effects used 250 to 500 mg of elemental magnesium a day for four to twelve weeks (Abbasi 2012; Mah and Pitre 2021; Zhang 2016). The UK reference intake is 300 mg a day for men and 270 mg for women from food; the NIH tolerable upper level from supplements is 350 mg a day, above which loose stools are the usual limit (NIH ODS). Judge sleep and mood at eight weeks of consistent daily dosing, blood pressure at twelve. A serum magnesium test after eight weeks rarely shows a change and is not the way to judge response.

## Forms compared

**Does the form change how long it takes?** It changes tolerance and absorption more than the timeline. Magnesium in citrate, chloride, lactate and aspartate forms is absorbed more completely than oxide and sulphate (NIH ODS). The amino acid chelate, bisglycinate (also called glycinate; they are the same compound), is the form usually chosen for sleep and calm because it does not loosen the bowel at usual doses, so the full dose reaches the tissues. Citrate and oxide are the forms chosen when a bowel effect is wanted. The trials above used oxide, citrate and bisglycinate; the direction of effect was the same, the difference is what you tolerate for eight weeks.

## Timing

**How quickly does magnesium start working?** The bowel responds first. Magnesium citrate, oxide, sulphate and hydroxide are osmotic, drawing water into the gut, and the effect on stools shows within 12 to 24 hours (NIH ODS). Nothing else moves that fast.

**How long does magnesium take to work for sleep?** Gradually. In the pooled trials of older adults with insomnia, sleep onset was around 17 minutes faster than placebo, a modest, real effect with evidence quality rated low (Mah and Pitre 2021). Some people notice a change in the first week; most measurable change is by weeks two to four; a fair verdict is at four to eight weeks. An eight-week trial at 500 mg a day improved insomnia scores alongside raised melatonin and lowered cortisol (Abbasi 2012).

**How long does magnesium take to work for anxiety?** Two to eight weeks at 200 to 400 mg a day, if it helps at all. The systematic review of 18 studies in anxiety-prone groups found the evidence suggestive of benefit but poor in quality, with the clearest effects where baseline intake was low and anxiety mild (Boyle 2017).

**How long for blood pressure?** Effects begin at about four weeks and reach their maximum by twelve weeks in the meta-analysis of trials (Zhang 2016).

**How long for heart rate variability?** One randomised trial found 400 mg a day over 90 days raised HRV in people under training load (Wienecke and Nolden 2016).

**What about cramps?** The 2020 Cochrane review of eleven trials concluded magnesium is unlikely to provide clinically meaningful cramp prevention in older adults; pregnancy evidence is conflicting; there are no trials in exercise cramps (Garrison 2020).

## Safety profile

Loose stools are the common dose-limiting effect and signal the dose or form should change. People with kidney impairment clear magnesium poorly and should not take supplemental magnesium without their clinician's agreement (NIH ODS). Magnesium is not a substitute for care for moderate or severe anxiety or insomnia; persistent insomnia belongs with a GP and CBT-I.

## Special populations

Older adults: the sleep trials were in people over 60 with insomnia and low intake, which is where the effect is clearest. Pregnancy: cramp evidence is conflicting; see the pregnancy magnesium page. Kidney impairment: supplemental magnesium only with clinical agreement. Long-term proton pump inhibitor users: absorption falls and deficiency can develop over years.

## Interactions

**What slows it down or blocks it?** Loose stools from citrate or oxide reduce the absorbed dose and are the usual reason people stop; switching to bisglycinate is the usual remedy. Proton pump inhibitors reduce magnesium absorption over long use; some diuretics increase urinary loss; alcohol increases urinary magnesium loss (NIH ODS; Pickering 2020). Magnesium reduces absorption of some antibiotics and bisphosphonates when taken together, so they are spaced by two hours (NIH ODS).

## Practical framework

Match the expectation to the reason: hours for the bowel, weeks for sleep and mood, three months for blood pressure. Take it in the evening if sleep is the aim, with food, at a dose that does not loosen the bowel. ## Common misconceptions

**Claim: if magnesium does not help sleep in a week, it does not work.** The trials measured change over four to eight weeks; a week is too early to judge.
