**How long does NAC take to work?** Weeks to months, because NAC acts by supplying cysteine for the cell to make glutathione, not by acting itself (2014 review). How it works

Cysteine is the rate-limiting building block of glutathione, and NAC is a stable way to deliver it; the 2014 review is explicit that NAC's direct antioxidant activity is minor and its value lies in intracellular conversion. Glutathione turns over on a scale of days but repletion of depleted stores takes weeks to months, which is why every trial that found a change ran for months.

Effective dose

What dose, and for how long? 600 mg once or twice a day is the range most supplement studies used and the 2021 review describes as well tolerated; the respiratory meta-analysis needed about 1,200 mg a day over long courses in people with airflow limitation, which is prescriber territory. Run three months before judging. Hospital acetylcysteine for paracetamol overdose is a different product and route (StatPearls) and has nothing to do with supplement dosing.

Does the form change how long it takes? No. NAC is sold as capsules, effervescent tablets and powder, all delivering the same molecule; the dose per day is what the trials varied, from 600 mg in most supplement studies to about 1,200 mg in the respiratory meta-analysis and 2.4 to 7.2 g a day of combined glycine and NAC in the GlyNAC trials. How quickly does NAC start working? Blood cysteine rises within hours of a dose, but that is not the measure that matters. Glutathione inside cells is what NAC exists to supply, and the GlyNAC trials show how slowly that moves: no overall rise after two weeks in 114 healthy older adults (an effect only in those who started low), improvements after 16 weeks in 24 older adults. When should I judge it? At three months, against the reason it was started. There is nothing to feel in the first week and no trial that measured one.

Safety profile

NAC at 600 to 1,200 mg a day is well tolerated in the trials, with nausea and digestive upset the usual complaints (2021 review). There is no UK upper level; very high intakes in the GlyNAC trials were reported safe over 16 weeks in older adults. It is not a substitute for prescribed care in any respiratory condition.

Older adults: the group in whom the GlyNAC trials found the clearest changes, and the only group with a 16-week placebo-controlled result. Anyone with a respiratory condition: prescriber-led, not a shop supplement. Pregnancy and breastfeeding: no supplement trials in the pool; take advice.

Interactions

What slows it down or changes the reading? Low glycine limits glutathione synthesis as much as low cysteine does in older adults, which is why NAC alone may show less than GlyNAC. NAC can thin mucus and has a sulphur smell; it is avoided with nitroglycerin because of reported headaches and blood-pressure effects, so anyone on cardiac tablets speaks to a pharmacist first.
