# How do you lower cholesterol naturally? What the evidence supports

## Summary

The evidence supports a set order. The first step is swapping saturated fat for unsaturated, away from butter, fatty meat, pastry and coconut oil toward olive and rapeseed oil, nuts, seeds and oily fish, which the NHS puts first. The second is soluble fibre that binds cholesterol, with oats and barley reaching 3 g of beta-glucan a day, psyllium at around 10 g a day, daily pulses, and plant sterol or stanol fortified foods. Total fibre reaches 30 g. Weight comes down where there is weight to lose, activity reaches 150 minutes a week, and smoking stops. Omega-3 lowers triglycerides rather than LDL. Where overall risk is high, this stacks with a statin rather than replacing it.

## How it works

Most cholesterol in the blood is made by the liver, not eaten, and saturated fat raises LDL by reducing the liver's clearance of it, which is why dietary cholesterol in eggs and prawns matters far less than the saturated fat in butter and pastry. Soluble fibres, beta-glucan from oats and barley, psyllium, the pectins in fruit and pulses, form a gel in the gut that binds bile acids so the liver must draw on cholesterol to make more. Plant sterols and stanols compete with cholesterol for absorption. Weight loss and exercise shift the whole lipid profile, lowering triglycerides and raising HDL, and stopping smoking restores HDL function. Omega-3 fats act on triglycerides through the liver's fat handling and do little to LDL.

## Timing

Dietary fat and fibre changes show in a lipid panel within four to six weeks, so the fair re-test is at three months. Psyllium effects appear within weeks. Weight and activity effects on triglycerides and HDL build over months. Stopping smoking lifts HDL over weeks to months.

## Safety profile

Chest pain, breathlessness on exertion, or pain in the calves on walking need a GP regardless of cholesterol. Very high cholesterol, total above 7.5 mmol/L or LDL above 4.9, or high cholesterol with a family history of early heart disease, raises the question of familial hypercholesterolaemia, which needs a GP and usually a prescription, not diet alone. Psyllium needs a full glass of water and is started gradually; it can reduce absorption of prescriptions taken at the same time, so separate by two hours. Plant sterols are not advised in pregnancy or for children. Nobody should stop a statin on the strength of a diet change without their GP.

## Special populations

People with diabetes have risk thresholds set lower and usually qualify for a statin regardless of cholesterol level. Post-menopausal women commonly see LDL rise and the fibre and fat levers apply in full. South Asian adults carry higher cardiovascular risk at a given cholesterol. Anyone with a family history of early heart disease should ask about familial hypercholesterolaemia testing rather than rely on diet.

## Guideline positions

NHS guidance on lowering cholesterol: cut saturated fat and replace it with unsaturated fat, eat at least 30 g of fibre a day, do 150 minutes of moderate activity a week, stop smoking, and cut alcohol. The British Dietetic Association notes that foods with added plant stanols and sterols have been shown to help where cholesterol is high, and the British Heart Foundation sets the oat and barley beta-glucan target at 3 g a day, roughly two to three portions. The 2018 meta-analysis of randomised trials found psyllium fibre lowered LDL, non-HDL cholesterol and apolipoprotein B. On cinnamon, a pooled analysis of 10 trials in type 2 diabetes found modest lipid reductions, while the Cochrane review judged the evidence insufficient. A cholesterol result is read with age, blood pressure, smoking and family history through a QRISK score, which is what decides whether lifestyle alone is enough.

## Practical framework

The first week covers the fat swap, which mostly means butter, fatty meat, cheese quantity, biscuits and pastry out, and olive or rapeseed oil, nuts and oily fish twice a week in. The second week builds the fibre, with a porridge or oat-based breakfast daily, pulses in at least one meal, psyllium husk from a teaspoon in water building to around 10 g a day, fruit and vegetables to five portions, and a sterol-fortified spread or yoghurt where cholesterol is clearly high. Alongside this, walking reaches 150 minutes a week, weight comes down where there is weight to lose, alcohol comes down, and smoking stops. The retest comes at three months. Omega-3 belongs here only where triglycerides are the problem. Whether three months of this has moved LDL enough, against the QRISK score, is the decision to take back to the GP. ## Common misconceptions

Saturated fat matters far more than the cholesterol in eggs. Coconut oil raises LDL. Fish oil capsules do not lower LDL. Red yeast rice is a statin in an unregulated dose rather than a natural alternative. A good diet does not cancel a high QRISK score, and the two conversations run together.
