How do you speed up recovery from an injury? What the evidence supports

Summary

Recovery time is set by the tissue, muscle in weeks, bone six to twelve, tendon three to six months, ligament up to a year, and nutrition's job is to stop you adding to it. Protein around 1.5 g per kilogram a day protects muscle through forced rest, the best-evidenced nutritional step. For tendon and ligament, 15 g of gelatin or collagen with around 50 mg vitamin C an hour before rehab exercise roughly doubled a collagen synthesis marker in a small crossover trial of eight men; promising, not proven. Sleep seven hours or more, eat enough in total, and correct iron or vitamin D if a blood test says so. The rehab drives the repair, and the nutrition supports it.

How it works

Injured tissue rebuilds by laying down collagen and, for muscle, contractile protein, and both processes are limited by blood supply, mechanical loading and amino acid availability. Muscle is well supplied and responds fast, which is why it is also lost fast when a limb is immobilised; adequate protein, and leucine-rich protein in particular, slows that loss. Tendon and ligament are poorly supplied, with baseline collagen turnover of around one to two percent a day, so nutrients only reach them well when exercise pushes blood through; that is the logic of the gelatin-before-rehab protocol, timing the amino acid supply to the window when loading has opened the tissue. Bone needs calcium, vitamin D and protein at adequacy. Sleep is when growth hormone and the repair programme run. None of it works without the loading the physiotherapist prescribes.

Timing

Protein matters from day one and throughout immobilisation. The collagen protocol is per rehab session, 30 to 60 minutes before, for the length of rehab. Tissue timelines do not compress: muscle strains weeks, fractures six to twelve, tendons three to six months, ligaments six to twelve. Judge progress on function and physiotherapy milestones, not on a supplement calendar.

Safety profile

Numbness, loss of pulse or colour below an injury, inability to bear weight, a deformity, or pain that escalates rather than settles are urgent assessments. Recurrent stress fractures or injuries that will not settle in athletes need a GP and a look at energy intake, bone density and hormones. Protein at 1.5 g per kilogram is safe in adults without kidney disease. Collagen and gelatin are food; the only cautions are allergy to the source animal and the lack of evidence in injured people. Anti-inflammatory painkillers used heavily may blunt tendon and bone repair, a conversation with a pharmacist or GP rather than a supplement decision.

Special populations

Athletes on restricted energy intake, women with disrupted cycles especially, recover badly and need intake corrected before anything else. Older adults lose muscle fastest during immobilisation and need the protein target most. Vegans can use pea protein and a vegan collagen-builder formula, noting the gelatin trial used animal gelatin. People with diabetes repair more slowly when glucose runs high.

Guideline positions

The ESPEN 2021 surgical nutrition guideline anchors the protein target of around 1.5 g per kilogram a day in adults recovering from surgical injury, the closest guideline analogue for traumatic injury. The collagen timing evidence is Shaw and colleagues 2017, a randomised double-blind crossover in eight healthy men: 15 g of vitamin C-enriched gelatin one hour before intermittent exercise roughly doubled the collagen synthesis marker P1NP versus placebo, which is a mechanism study in a small healthy group rather than a trial of injured people. Practical framework

Common misconceptions

No supplement makes a tendon repair in a month. Collagen capsules at a few hundred milligrams bear no relation to the 15 g the trial used. Resting completely is not faster; loaded rehab is what drives repair. And vitamin C megadoses do nothing for tissue that was not deficient.
