# hs-CRP blood test results explained: what your number means

## Summary

A UK hs-CRP result reads against the CDC and AHA bands. Below 1 mg/L is lower cardiovascular risk, 1 to 3 average, above 3 higher, and 10 or more an acute process to be repeated once well. One reading is provisional, because CRP rises for days after a cold, an injury, a vaccine, dental work or a hard session, and the consensus is the average of two tests at least two weeks apart, taken when well. hs-CRP is not in QRISK3, so NICE does not use it to decide statins, and it is read alongside. A persistently raised result points at central weight, insulin resistance, short sleep, smoking and gum disease, and it distorts ferritin upward.

## How it works

Immune cells release interleukin-6 in response to infection, injury or the low-grade signalling from visceral fat, and the liver answers with CRP within hours. The standard CRP test detects the tens to hundreds of mg/L of infection; the high-sensitivity assay resolves the 0.3 to 10 range where background inflammation sits. That background is driven mostly by fat around the organs, which secretes inflammatory signals continuously, and by insulin resistance, short sleep, smoking, gum disease and chronic stress. CRP has a half-life of about 19 hours, so it climbs and falls fast, which is why a reading taken three days after a cold says nothing about baseline. The same inflammatory signalling pushes ferritin up and iron into storage, which is why a raised CRP changes how a ferritin result is read.

## Timing

CRP half-life is about 19 hours; an acute rise resolves within one to two weeks of recovery. Baseline is the average of two results at least two weeks apart. Lifestyle effects show over 8 to 12 weeks; re-test every few months at most. No fasting needed.

## Safety profile

A CRP of 10 or more with fever, pain, a swollen joint, unexplained weight loss or night sweats is a GP matter now, not a lifestyle matter. A persistently raised hs-CRP with joint pain, bowel symptoms, a rash or fatigue that does not lift is assessed for an inflammatory condition. ## Special populations

Women, older adults, pregnant women and those on oestrogen-containing contraception or HRT run higher baseline values. People with obesity commonly sit above 3 from visceral fat alone. Anyone with a known inflammatory condition reads CRP with their specialist. Smokers run higher until they stop.

## Guideline positions

CDC and AHA 2003 workshop report: hs-CRP tiers of below 1 mg/L, 1 to 3, and above 3 for cardiovascular risk; use the average of two independent measurements at least two weeks apart; if the concentration is 10 mg/L or more, repeat the test and examine for sources of infection or inflammation; fasting or non-fasting is acceptable and time of day is not significant. Whatever the band, the first act is the second test, at least two weeks later, when well, with no hard training for two days beforehand. The number is then read with its company, meaning waist, HbA1c, triglycerides and HDL, sleep, smoking, gums and ferritin, because a CRP above 3 with a ferritin of 60 can still be iron deficiency. ## Common misconceptions
