Is leaky gut real? What the evidence says

Summary

Intestinal permeability is real and raised in coeliac disease, inflammatory bowel disease, after gut infections, with heavy alcohol and some anti-inflammatory drugs, and is managed by managing the condition. Leaky gut syndrome as a cause of fatigue, brain fog, skin, joint and autoimmune problems in otherwise well people is not a recognised diagnosis and has no validated test, and the kits and stool panels sold for it are not clinically useful. The symptoms on the list have known causes worth testing, meaning ferritin, B12, vitamin D, TSH, HbA1c, a coeliac test, and IBS or mood assessment. How it works

The gut lining is one cell thick and held together by tight junctions that open and close to let nutrients and immune signals through; some permeability is normal and is how the gut samples what passes. Coeliac disease, inflammatory bowel disease, infection, alcohol and NSAIDs damage the lining or loosen the junctions measurably, and the body's response is inflammation in the gut and, sometimes, systemic. Short-chain fatty acids from fermented fibre feed the lining cells and tighten the junctions, which is the mechanism behind fibre's protective effect. What the leaky gut syndrome model adds, that food proteins and toxins cross into the blood in well people and cause distant symptoms, has not been shown, and the marker tests sold for it have not been validated against any outcome.

Timing

Blood results within a week. Fibre effects on gut output within weeks. A coeliac test needs six weeks of gluten beforehand. Symptoms that persist after a normal panel warrant a GP review, not a longer protocol.

Safety profile

Going gluten-free before a coeliac test makes the test useless. Blood in stool, weight loss, persistent diarrhoea, night symptoms or anaemia needs a GP. Common misconceptions
