# Liver support supplements: what the evidence shows

## Summary

Choline is the only common liver-supplement ingredient with authorised GB and EU claims, contributing to the maintenance of normal liver function and normal lipid metabolism, with adequate intake set at 400mg daily for adults. The claims were substantiated by fatty-liver changes in people fed choline-free nutrition, reversed when choline was restored. The botanical ingredients in liver formulas, artichoke included, hold no authorised liver claims; their evidence is mechanistic. No supplement detoxes the liver; the liver detoxifies itself, and nutrition supports the machinery rather than replacing it.

## How it works

The liver runs two detoxification phases: phase I enzymes transform compounds, and phase II conjugation makes them excretable. Choline is required to export fat from the liver as phosphatidylcholine in lipoproteins; without enough, fat accumulates, which is the observation behind the authorised claims. Artichoke cynarine is associated with bile production, the route through which processed waste leaves. The polyphenols and tocotrienols provide antioxidant cover for hepatocytes doing chemically demanding work.

## Effective dose

EFSA sets adequate choline intake at 400mg daily for adults, rising to 480mg in pregnancy and 520mg when breastfeeding, and most UK diets undershoot it. The authorised choline claims apply at qualifying intakes, so a formula providing 33mg per capsule is a contributor alongside dietary choline from eggs, meat and soya rather than a full answer; a standalone choline supplement is the route to claim-relevant doses. Artichoke trial doses for digestive comfort have ranged widely, and no authorised dose exists to anchor on.

## Forms compared

Liver formulas are typically capsule blends built around a bile-supporting botanical plus antioxidants and choline. The Liver Support capsule here provides 116.5mg artichoke extract standardised to 2.5% cynarine, 33mg choline, 83mg green tea extract (45% EGCG), 67mg quercetin, 67mg turmeric (95% curcuminoids) with 666mcg piperine, plus 8.2mg mixed tocotrienols, vitamin C, astaxanthin, squalene and phytosterols. Standalone choline products supply the larger doses relevant to the authorised claims.

## Timing

Liver formulas are best taken with food, both for tolerability and because several components, curcuminoids, tocotrienols and astaxanthin among them, are fat soluble and absorb better with a meal. Choline can be taken any time. There is no evidence that timing within the day changes outcomes for any ingredient in this class; consistency over weeks is what matters. Green tea extract is mildly caffeinated, so sensitive people may prefer earlier in the day.

## Safety profile

At label doses these ingredients are well tolerated. The cautions that matter: green tea extract is the ingredient class regulators moved on, hence the EU EGCG cap that this formula sits well under; artichoke is a thistle-family plant, so ragweed-family allergies can cross-react; and high-dose choline above around 3.5g daily causes a fishy odour and hypotension, orders of magnitude above supplement doses. None of this replaces investigation of actual liver symptoms, which are GP territory from the first appointment.

## Special populations

Pregnancy raises choline needs to 480mg daily and breastfeeding to 520mg, and choline in pregnancy has its own authorised claim territory around infant liver function, but green-tea-containing blends are not the pregnancy route; a midwife or GP should agree any supplement routine in pregnancy. People who avoid eggs and meat get less dietary choline and have more reason to attend to intake. Anyone with existing liver disease should make all supplements a clinical conversation first.

## Interactions

Green tea extract at high doses has been linked to rare liver injury, which is why EU law caps EGCG in supplements; the 37mg EGCG here sits far below the 800mg daily cap. Anyone with diagnosed liver disease, on prescribed therapy, or with symptoms such as jaundice, dark urine or persistent right-sided pain needs a GP, not a supplement. Grapefruit-style enzyme interactions are not established for these ingredients at label doses. ## Guideline positions

Authorised GB and EU Article 13.1 claims: choline contributes to the maintenance of normal liver function, to normal lipid metabolism, and to normal homocysteine metabolism. EFSA adequate intake for choline is 400mg daily for adults (2016). Artichoke, milk thistle and other botanicals hold no authorised liver claims; botanical claims remain on hold in the GB and EU registers. EU Regulation 2022/2340 caps green tea EGCG in supplements at 800mg daily.

## Practical framework

Read any liver product by asking two questions: how much choline does it provide toward the 400mg adequate intake, and are the botanicals framed as support or as detox. Choose a blend like Liver Support when you want artichoke, antioxidants and a choline contribution together, and add dietary choline, eggs are the densest common source, toward the daily figure. Choose standalone choline when the liver-function claim itself is the target. Anything promising to flush, cleanse or reset a liver is selling a mechanism that does not exist.

## Common misconceptions

Liver detoxes and cleanses are marketing constructions; the liver detoxifies continuously and cannot be flushed by a product. Milk thistle being absent from a formula is not a gap, because silymarin holds no authorised claim and its clinical trial record is mixed. And a liver formula does not manage fatty liver disease; weight, alcohol and metabolic health drive that condition, and diagnosed disease belongs with a GP, with nutrition supporting alongside.
