# Supplements for heart health: what the evidence supports

## Summary

The outcome-evidenced layer is not supplements, meaning not smoking, blood pressure known and managed, the Mediterranean pattern that PREDIMED tested, 150 minutes of activity, weight, and, where QRISK3 puts risk above threshold, the statin conversation with the GP per NICE NG238. On the shelf, plant sterol and stanol fortified foods genuinely lower LDL at labelled doses, oat and psyllium fibre lower it modestly, and omega-3 at 2 to 4 g is a triglyceride tool rather than heart-attack protection at ordinary doses. Vitamin E, garlic for outcomes and general CoQ10 prevention did not deliver in trials. The actual numbers come first, on the cholesterol hub, then the levers.

## How it works

Cardiovascular events grow from decades of LDL particles crossing into artery walls, inflammation around what accumulates, and blood pressure stressing the structure, which is why the effective interventions act on LDL, pressure and inflammation over years rather than on any acute feeling. Statins cut LDL production at the liver; sterols and stanols block cholesterol absorption in the gut, which is why they only work taken with meals at sufficient dose; soluble fibre binds bile acids so the liver spends cholesterol replacing them; omega-3 at gram doses clears triglycerides through the liver. Nothing in that chain can be felt working, which is why heart supplementation without a lipid panel and a blood pressure reading is guesswork in both directions.

## Timing

Sterols and fibre move LDL within four to six weeks. Omega-3 needs eight to twelve for triglycerides. Dietary pattern effects on the panel show at three months. Outcome benefits are measured in years, which is why the numbers stand in for the future.

## Safety profile

Chest pain, breathlessness on exertion, or palpitations with faintness are for the GP or 999, not the supplement aisle. Nobody stops a statin, blood pressure prescription or anticoagulant because a supplement started; omega-3 at gram doses is reviewed with the GP on blood thinners. Red yeast rice is an unregulated statin in disguise and is not a safe workaround. Nutri365 checks for interactions and contraindications, heart prescriptions above all, before suggesting anything.

## Special populations

A family history of early heart disease or a total cholesterol above 7.5 raises the familial hypercholesterolaemia question, on its [own page](/apps/learn/cholesterol-above-7-5-or-ldl-above-4-9-familial-hypercholesterolaemia-check), and that is specialist territory where food tools are adjuncts. People with diabetes run higher baseline risk and their thresholds differ. Post-menopausal lipid shifts are read on the [perimenopause diet page](/apps/learn/perimenopause-diet-what-the-evidence-supports).

## Guideline positions

NICE NG238, read across the [cholesterol hub](/apps/learn/cholesterol-blood-test-results-explained-uk-what-your-number-means): QRISK3 assessment, atorvastatin 20 mg offered at 10 percent ten-year risk, LDL and non-HDL targets on therapy. The triglyceride role of omega-3 and the REDUCE-IT context sit on the [triglycerides page](/apps/learn/triglycerides-above-1-7-what-it-means). The dietary pattern evidence, PREDIMED's republished outcome trial included, is on the [anti-inflammatory diet page](/apps/learn/anti-inflammatory-diet-what-the-evidence-supports). The full food-first LDL toolkit, sterols, oats, fibre doses, is on [how to lower cholesterol naturally](/apps/learn/how-to-lower-cholesterol-naturally-what-the-evidence-supports).

## Practical framework

Numbers come before capsules, meaning a lipid panel and a blood pressure reading, both cheap and both silent until known, with results read on the [cholesterol hub](/apps/learn/cholesterol-blood-test-results-explained-uk-what-your-number-means) and pressure against the home-monitoring routine the GP practice recommends. The outcome layer runs next, meaning smoking gone, 150 minutes of activity, the Mediterranean pattern, and weight where relevant via [how to lose weight sustainably](/apps/learn/how-to-lose-weight-sustainably-what-the-evidence-supports). The LDL food tools apply where the panel says so, meaning sterol or stanol fortified foods at labelled dose with meals, and oats and psyllium daily, per [how to lower cholesterol naturally](/apps/learn/how-to-lower-cholesterol-naturally-what-the-evidence-supports). Omega-3 runs at 2 to 4 g only for the high-triglyceride pattern, per the [triglycerides page](/apps/learn/triglycerides-above-1-7-what-it-means). The QRISK3 and statin conversation happens with the GP where risk or family history says so, and CoQ10 applies only in the specific statin-muscle-symptom context the GP agrees to trial. The panel is retested at three months after any change. Nutri365 reads whether the numbers move with the oats, the pattern or the prescription, checks for interactions and contraindications, heart prescriptions above all, and refers questions outside scope to a health specialist.

## Common misconceptions

Fish oil at one capsule a day did not cut heart attacks in the trials. Garlic and vitamin E did not deliver. CoQ10 is not general heart insurance. Dietary cholesterol matters less than saturated fat for most people. Feeling fine is not a cardiovascular measurement.
