# Total vs active B12: what your test result actually measures

## Summary

The two tests measure different things. Total serum B12 counts all circulating cobalamin, most of which is bound to a carrier protein that cells cannot take vitamin from; active B12 counts only the transcobalamin-bound fraction that is actually available for cellular uptake. UK guidance reads total B12 below 180 nanograms per litre as deficiency likely, 180 to 350 as an indeterminate zone that needs a follow-up marker such as methylmalonic acid, and active B12 below 25 picomoles per litre as deficient. The practical consequences: a total result in the indeterminate zone settles nothing on its own, and testing before you supplement matters, because any B12 supplement raises the numbers and muddies a later diagnosis.

## How it works

Circulating B12 rides two carriers. Most of it, roughly four fifths, is bound to haptocorrin, a protein with no known delivery role: cells cannot take vitamin from it. The remainder rides transcobalamin, and only this holotranscobalamin fraction docks with cell receptors for uptake, which is why it earns the name active B12 (Nexo 2011). A total serum B12 result therefore mostly measures vitamin the body cannot currently use, which is the root of every interpretation problem this page covers.

## Effective dose

This is an interpretation page, not a dosing page, and the order matters: test first, supplement second. Any oral B12 raises serum values within days regardless of whether tissue stores were the problem, so supplementing before testing spends the clean baseline. ## Forms compared

The choice between tests is often made by the lab, not by you: many UK labs run total B12 first, with active B12 or methylmalonic acid as follow-ups. If you are commissioning your own panel and symptoms are the reason, an active B12 gives fewer false reassurances than a total alone, at the cost of availability. ## Timing

A morning sample is fine and fasting is not usually required for B12 itself, but timing relative to supplementation is everything: stop the multivitamin conversation with your clinician before the draw, because recent supplementation is the commonest reason a result cannot be interpreted. Retesting after a change runs on months, not weeks.

## Safety profile

The risk in this topic is interpretive, not chemical: a normal-range total result can close the case wrongly while a real cellular deficiency continues, and neurological features of B12 deficiency do not wait politely. Numbness, tingling, balance problems or cognitive change alongside any borderline result make the GP conversation urgent rather than optional.

## Special populations

People on metformin or long-term acid suppression, adults over 65, vegans, and anyone after gastric surgery are the groups where the total test misleads most often, because absorption-driven deficiency develops slowly against a normal-looking total. Pregnancy shifts the carrier proteins themselves, so pregnancy results need clinical interpretation rather than this page's bands.

## Interactions

Interpretation confounders rather than supplement clashes: recent B12 supplementation inflates both tests; folate status interleaves with B12 in the same metabolic pathway, which is why the two are usually tested together; and a high folate intake can mask the blood picture of B12 deficiency while neurological risk continues, covered in the folate entry.

Nexo 2011: holotranscobalamin is the fraction of circulating cobalamin available for cellular uptake and serves as a marker of B12 status with distinct analytical properties. ## Practical framework

Read your own report in three steps. One, identify which test you actually had: units are the giveaway, nanograms per litre for total, picomoles per litre for active. Two, place the number: total below 180 or active below 25 is a GP conversation about cause, not a supplement decision; total 180 to 350 is unfinished business that a second marker settles, so push for the follow-up rather than accepting reassurance. Three, protect the baseline: if you have not yet supplemented, test before you start, and if you already supplement, say so on the form because it changes what the numbers can mean. ## Common misconceptions

That a normal total B12 rules deficiency out: most of what it counts is unavailable to cells, and the indeterminate zone exists precisely because the total is a blunt instrument. That active B12 is a niche test: it measures the only fraction cells can use, and UK guidance gives it its own threshold. That supplementing first and testing later loses nothing: it costs the diagnosis, because raised numbers after supplementation cannot distinguish a corrected gap from a gap that was never there.
