# Vitamin D at 15 nmol/L: what does a deficient result mean?

## Summary

Vitamin D below 25 nmol/L is deficient by UK standards. The response runs on two tracks. Where symptoms are present, meaning bone or muscle aches or weakness, or where risk factors apply, the GP route matters, because symptomatic deficiency is managed with a prescribed loading course totalling around 300,000 IU over 6 to 10 weeks, followed by 800 IU daily. Without symptoms, daily supplementation at maintenance doses begins the climb, and 800 to 1,000 IU daily typically adds roughly 24 to 29 nmol/L by the three-month steady state. On either track, retesting before three months reads nothing useful.

## How it works

Below 25 the storage pool is too small to reliably supply the active hormone, and calcium absorption falls measurably. The body responds by drawing on bone, which is the mechanism behind the aches and, where the state is sustained, behind osteomalacia in adults and rickets in children. Muscle tissue also carries vitamin D receptors, which is why proximal weakness, meaning difficulty with stairs and with rising from chairs, belongs to this band. At UK latitude a level this low usually means the summer top-up did not happen, through indoor life, covered skin, darker skin or malabsorption.

## Timing

Steady state on a daily dose arrives at around three months, which makes that the earliest meaningful retest. From 15, maintenance-dose arithmetic reaches the sufficient line in one to two testing cycles, and a prescribed loading course reaches it in weeks, which is why symptomatic cases route through the GP. Once above 50, the [sufficient band](/apps/learn/vitamin-d-60-sufficient-what-that-means-for-winter) takes over with the winter maintenance rules.

## Safety profile

A loading course should not be assembled from retail products, because the high-dose correction pathway is clinical territory, with calcium and context needing monitoring. Bone pain, fractures from minor trauma, or marked muscle weakness are reasons to see a GP now rather than supplement questions. Granulomatous disease, such as sarcoidosis or TB, means specialist advice first. The adult ceiling of 4,000 IU daily without advice stands even during a climb from deficiency.

## Special populations

Darker-skinned adults at UK latitude reach this band most easily and carry the year-round supplementation advice thereafter. Older and housebound adults combine reduced skin synthesis with lower intake, and fracture risk makes correction genuinely consequential for them. Anyone with coeliac disease, IBD or another malabsorption condition should involve their GP, because standard doses may underperform. Pregnancy at this level belongs in the midwife conversation. The granulomatous-disease rule holds most firmly here, and sarcoidosis or TB means specialist advice before any supplementation.

## Guideline positions

The UK framework reads below 25 nmol/L as deficiency outright. With symptoms or high-risk context, NICE-aligned practice uses a loading regimen totalling around 300,000 IU over 6 to 10 weeks in prescribed weekly or split doses, followed by maintenance at 800 IU daily. Without symptoms, maintenance dosing without loading is the standard start. The expected pace on maintenance is roughly 24 to 29 nmol/L gained by the three-month steady state per 800 to 1,000 IU daily.

## Practical framework

The response runs on two tracks. Anyone symptomatic, carrying risk factors, or simply below 25 and unsure books the GP appointment, because the loading course that corrects symptomatic deficiency efficiently is prescribed and monitored, and arriving with the result speeds it. Anyone asymptomatic and starting the climb takes a daily D3 dose in the 800 to 2,000 IU range, with [Vitamin D3 & K2 Oil](/products/vitamin-d3-k2-oil) the drop-form option, taken with a meal containing fat, alongside the diet's modest contributions from oily fish, eggs and fortified foods. The retest comes at three months rather than before, expecting the maintenance-pace gain, and the [full scale](/apps/learn/vitamin-d-blood-test-results-explained-uk-what-your-number-means) shows the direction of travel. Nutri365 reads the result against season, symptoms and history over time, checks for interactions and contraindications, and refers questions outside scope to a health specialist.
