# Vitamin D at 250 nmol/L: what should you do now?

## Summary

Vitamin D above 220 nmol/L is too high. This is the territory clinical references associate with toxicity risk, driven by raised calcium, and it essentially only arises from sustained high-dose supplementation. Two steps apply. All vitamin D supplementation stops now, including multivitamins and fortified extras, and a GP review is booked, where a calcium check is the likely first step. Prompt attention is needed if raised-calcium symptoms are present, meaning thirst, passing urine frequently, nausea, constipation or confusion. The level falls over months once intake stops, and retesting and any restart decisions belong with the GP.

## How it works

At these levels the storage pool is large enough that conversion keeps the active hormone and calcium absorption high regardless of need, and calcium is the axis on which vitamin D toxicity turns. Hypercalcaemia, meaning raised blood calcium, produces the thirst, frequent urination, gut symptoms and confusion of the classic picture, and where sustained it can involve the kidneys. Sun exposure cannot produce this, because the skin self-limits. This is a supplement phenomenon, typically from sustained intakes far above the 4,000 IU ceiling.

## Timing

Stored vitamin D clears slowly, and a level here takes months to descend through the high bands toward the normal range, with the retest schedule set clinically. The eventual restart question, of whether and at what dose, is a GP conversation informed by how the level was reached.

## Safety profile

Certain symptoms upgrade this from prompt to urgent. Marked thirst and frequent urination, nausea or vomiting, constipation, bone pain, muscle weakness, drowsiness or confusion alongside a known very high level warrant same-day clinical contact. In the absence of symptoms this remains a stop-and-review rather than a watch-and-wait, because the level only falls once intake stops and the calcium question is answered in one blood draw.

## Special populations

Nearly everyone in this band is a high-dose supplement user, sometimes through imported or mislabelled products whose actual content exceeded the label, which is worth mentioning to the GP. Anyone here on prescribed vitamin D contacts the prescriber promptly rather than simply stopping, because context matters. Granulomatous disease at this level is urgent specialist territory. Anyone with the raised-calcium symptom cluster is in same-day territory.

## Guideline positions

Clinical references place toxicity concern at sustained very high intakes with 25-hydroxyvitamin D levels above roughly 220 nmol/L, and the UK adult ceiling of 4,000 IU daily exists to keep supplementation far from this territory. No framework reads a level here as a goal. The guidance-aligned response is cessation and clinical review, with serum calcium the natural first check.

## Practical framework

The plan is short by design. Every vitamin D source stops today, meaning capsules, sprays, drops, multivitamins and fortified shakes. A list of what was being taken, with doses, makes the GP appointment productive. The review is booked, a calcium check is expected, and the clinical side owns retesting and any eventual restart at sensible doses. The level falls over months rather than days, which is normal and not a reason for further action beyond what the GP sets.
