# Vitamin D at 45 nmol/L: nearly there, or still low?

## Summary

Vitamin D at 45 nmol/L is still insufficient by UK standards, though only just, with sufficiency at 50. Correction from here is the easiest on the scale, because the standard daily maintenance dose crosses the line comfortably inside one three-month cycle, and the NHS winter advice of 10 µg daily exists to stop a 45 in autumn becoming a 25 by February. The season changes the reading. A 45 in early spring is a winter trough likely to self-correct through summer, while a 45 in September is a summer peak that will not survive winter unsupplemented.

## How it works

At 45 the storage pool functions adequately day to day, and the issue is trajectory and margin. UK winters drain a broadly similar amount regardless of the starting point, so the September number predicts the February one. A summer peak of 45 typically falls to the low 30s or below by late winter, which is how a nearly sufficient autumn becomes an insufficient spring.

## Timing

Three months to steady state means a spring start reads sufficient by summer, and an autumn start holds the line through winter, which is the more valuable direction. The same-season annual test provides the trend that matters, and testing more often than that provides little, because vitamin D moves slowly.

## Safety profile

The standing rules are the only ones needed. The ceiling is 4,000 IU daily without advice, all sources are counted, and granulomatous conditions go to a specialist first. There is no case at 45 for high-dose products or loading-style intakes, because the gap is five points and a maintenance dose clears it.

## Special populations

Anyone in a risk group, meaning darker skin, covered skin, older age, being housebound, or malabsorption, should read 45 as a supplemented level to protect rather than a level to coast on. Athletes with winter training blocks have a practical interest in avoiding the winter trough. Pregnancy carries the standard daily advice regardless of a 45. The granulomatous-disease rule is unchanged, with specialist advice before supplementing.

## Guideline positions

The framework reads 38 to 50 as the top of insufficiency, managed like the rest of the 25 to 50 band, meaning maintenance dosing and no loading, with 800 to 1,000 IU daily adding roughly 24 to 29 nmol/L by three months, which places the 50 line one cycle away. The NHS winter advice of 10 µg daily from October to March for everyone is aimed at exactly the seasonal drop this band is exposed to.

## Practical framework

One habit closes the gap and holds it. A daily D3 dose of 800 to 1,000 IU is ample from here, taken with a meal containing fat, with [Vitamin D3 & K2 Oil](/products/vitamin-d3-k2-oil) the drop-form option, and run through every winter. Summer sun then contributes through sensible exposure without burning. Retesting is optional at this band, and the same-season annual check is the simplest version. The [full scale](/apps/learn/vitamin-d-blood-test-results-explained-uk-what-your-number-means) covers the neighbouring bands. Nutri365 reads the result against season and history over time, checks for interactions and contraindications, and refers questions outside scope to a health specialist.
