# Vitamin D blood test results explained: what does your number mean?

## Summary

Reading a UK vitamin D result takes three thresholds. Below 25 nmol/L is deficient and warrants correction, with clinician-managed loading where symptoms are present. From 25 to 50 is insufficient, managed with a daily maintenance dose and no loading. Above 50 is sufficient for bone health. Units matter, because UK reports use nmol/L and American sources use ng/mL, and ng/mL multiplied by 2.5 gives nmol/L. UK winter sun is too weak for skin synthesis, so the NHS advises 10 µg daily for everyone from October to March, and results drawn in late winter read lower than the same person in September. Toxicity is rare and sits far above the normal range.

## How it works

Serum 25-hydroxyvitamin D measures the storage form of vitamin D, the pool the body draws on to make the active hormone, which is why it is the agreed status marker. The pool fills from two sources, meaning UVB on skin through the British summer, which supplies 80 to 90 percent, and diet plus supplements for the rest. Skin synthesis stops almost entirely from October to March at UK latitude, so the pool drains through winter, and this seasonality is why a January reading of 35 and a September reading of 60 can belong to the same person.

## Timing

Vitamin D moves slowly. A new daily dose reaches its steady state at around three months, which makes earlier retesting uninformative. The season shapes the reading, because a spring result shows the winter trough and an autumn result the summer peak, and the trough is the number that matters for the NHS winter advice. Annual testing at the same point in the year gives the cleanest trend.

## Safety profile

Two safety rules frame the scale. Loading regimens for deficiency are clinical territory, individually dosed and monitored, and no page here teaches self-loading. At the top end, sustained high-dose supplementation is the usual route to excessive levels, which is why the adult ceiling of 4,000 IU daily without advice exists, and granulomatous conditions need specialist input before any supplementation. Vitamin D from sun exposure cannot reach toxic levels, and supplements can.

## Special populations

Risk runs higher, and year-round supplementation advice applies, for people with darker skin at UK latitude, anyone covering skin outdoors, older adults and the housebound, and people with malabsorption or on certain long-term prescriptions a GP will recognise. Pregnancy carries its own standard advice. One group runs in the other direction, because anyone with sarcoidosis, TB or another granulomatous disease needs specialist advice before supplementing at all, since their handling of vitamin D can raise calcium.

## Guideline positions

The UK framework reads deficient below 25 nmol/L, insufficient from 25 to 50, managed with maintenance dosing without loading, and sufficient above 50. Symptomatic deficiency is corrected with a clinician-managed loading course totalling around 300,000 IU over 6 to 10 weeks, followed by 800 IU daily, and that pathway belongs with a GP. Population advice sits alongside, meaning 10 µg (400 IU) daily for everyone from October to March, year-round for at-risk groups, and an adult ceiling of 4,000 IU daily not to be exceeded without advice.
