How it works

B12 is the cofactor for methylmalonyl-CoA mutase, the enzyme that converts methylmalonyl-CoA into succinyl-CoA. When cells lack usable B12 the conversion stalls, methylmalonyl-CoA is hydrolysed to free methylmalonic acid, and MMA rises in the blood. That is why MMA is called a functional marker. Serum B12 counts what is circulating; MMA reports whether the B12-dependent machinery inside cells is actually running. MMA is also more specific to B12 than homocysteine, which rises in folate and B6 deficiency as well.

Effective dose

MMA is a test, not a supplement, so the dose question here is about what testing decisions follow. A raised MMA with low or borderline B12 supports genuine deficiency and routes to the GP-led pathway, where the cause determines whether oral B12 or injections apply. A raised MMA with normal B12 does not have a supplement answer, because the likelier explanations are renal clearance or other metabolism, and that result belongs with a GP.

Forms compared

MMA is usually measured in serum or plasma. A urinary MMA-to-creatinine ratio exists and is less affected by kidney function, but serum MMA is what UK labs typically run in the B12 workup.

Timing

Test before supplementing. Any B12 supplement restores mutase activity and lowers MMA within weeks, which is exactly what you want after a confirmed deficiency and exactly what you do not want before one, because it can normalise the result while the underlying cause goes unexamined. If a B12 workup is planned, hold off starting B12 and mention anything already being taken.

Safety profile

The test itself is a standard blood draw. The interpretive risk sits in over-reading a raised MMA as proof of B12 deficiency when kidney function is reduced, and in under-reading a normal-B12-raised-MMA pattern that deserves a renal look. Both readings belong with a GP rather than a supplement decision.

Special populations

MMA runs higher with age even without deficiency. In a general-population study of adults aged 60 to 75, the median MMA was around 170 nmol/L, and both lower B12 and lower kidney function independently pushed MMA up. Older adults, and anyone with known kidney impairment, need their MMA read against that context rather than a young-adult expectation.

Interactions

B12 supplements in any form lower MMA. Reduced kidney function raises it. Metformin and proton pump inhibitors, which lower B12 absorption over long-term use, can indirectly raise MMA by depleting B12, which is one reason long-term users of either drug feature in UK testing guidance.

NICE NG239 (2024) positions MMA as the second-line marker when total serum B12 falls in the 180 to 350 ng/L indeterminate zone, alongside clinical judgement, with active B12 below 25 pmol/L reading as deficient. No UK-wide MMA reference range is set. Laboratory ranges vary, with common adult upper cutoffs in the region of 0.27 to 0.45 micromoles per litre, and the issuing lab's range is the one that applies.

Common misconceptions

A raised MMA does not automatically mean B12 deficiency, because reduced renal clearance raises MMA with no B12 problem at all. A normal MMA does not always end the story either, since a small number of people with classic deficiency features respond to B12 despite normal markers, which is why UK practice pairs the numbers with the clinical picture. And there is no universal cutoff. Quoting a single MMA threshold without naming the lab range behind it is a sign the source is oversimplifying.
