Both raise B6 status, and head-to-head human superiority evidence for P5P is thin, so the honest case for P5P rests elsewhere: the documented sensory neuropathy from sustained high-dose pyridoxine, which laboratory work links to pyridoxine competitively inhibiting the active P5P form. Dose is the bigger decision than form. The NHS advises not exceeding 10mg daily long term unless a doctor advises it, and EFSA lowered its adult ceiling to 12mg daily in 2023.
Pyridoxal 5-phosphate is the working coenzyme for well over one hundred enzymes covering amino acid, neurotransmitter and glycogen chemistry. Swallowed pyridoxine must be phosphorylated and oxidised to become P5P; swallowed P5P is dephosphorylated in the gut and rebuilt inside cells. Laboratory work on pyridoxine toxicity found pyridoxine itself competitively inhibiting P5P-dependent enzymes, which is the proposed reason B6 excess mimics B6 deficiency in the nerves.
The UK reference intake is 1.4mg daily for men and 1.2mg for women, met by most diets. Chronic high-dose pyridoxine, grams per day over months in the classic reports and lower doses in some case series, causes progressive sensory neuropathy: numbness, tingling and unsteady gait, usually improving after stopping. Any new numbness or tingling while supplementing B6 is a stop-and-see-your-GP signal, whatever the form and dose.
People on isoniazid, hydralazine or penicillamine, and those with kidney disease or malabsorption, run lower B6 and belong under a GP's guidance. In pregnancy, B6 use for nausea sits with a midwife or GP, and the EFSA ceiling applies unchanged.
