Summary

On a fully plant-based diet, vitamin B12 has to come from fortified foods or a supplement, because the vitamin is made by microbes and stored in animal tissue, not in plants. The testing logic runs in this order: test before you begin supplementing where possible, because any B12 supplement raises blood values within days and clouds a later diagnosis. The UK thresholds then apply as for anyone: total B12 below 180 nanograms per litre reads as deficiency likely, 180 to 350 is indeterminate and warrants a second marker, and active B12 below 25 picomoles per litre reads as deficient. How it works

Vitamin B12 is synthesised by bacteria and accumulates up the food chain into animal tissue, which is why unfortified plant foods carry no reliable amount. The body holds substantial liver stores, typically enough for years, so a new vegan can feel fine for a long time while stores quietly run down. That lag is the trap: symptoms arrive late, and the neurological ones do not always announce themselves as a diet problem.

Effective dose

The arithmetic from the NIH Office of Dietary Supplements explains the odd-looking dose options. Absorption through intrinsic factor saturates at roughly 1.5 to 2 micrograms per sitting, and beyond that only about 1 to 2% of a large dose gets through passively. So either pattern works: small amounts several times a day through fortified foods, or a larger daily or few-times-weekly supplement whose passive fraction covers the requirement. The UK reference intake is 1.5 micrograms a day; common vegan supplement practice uses far larger labelled doses precisely because so little of each dose is absorbed.

Forms compared

Which test matters more than which supplement here. Total serum B12 counts all circulating cobalamin, most of it bound to a carrier cells cannot draw from; active B12 counts the fraction actually available for uptake, and gives fewer false reassurances in people whose intake is marginal. Timing

Any time of day, with or without food, since supplemental B12 is free cobalamin and does not depend on stomach acid the way food-bound B12 does. The timing that matters is test-before-supplement: a baseline taken after months of supplementing cannot distinguish a corrected gap from one that was never there.

Safety profile

No UK or EU upper level is set for B12 and excess is excreted, so the safety question sits with the untreated gap, not the supplement. B12 deficiency can injure nerves before it shows in a blood count, and neurological features, numbness, tingling in hands or feet, balance problems, memory change, make the GP conversation urgent rather than routine. Diet-related deficiency confirmed by testing is corrected under GP guidance, and non-dietary causes need GP-led care whatever the diet.

Special populations

Long-term vegans and vegetarians with low fortified-food intake are the core group. Interactions

Interpretation confounders more than clashes: recent supplementation inflates both tests; high folate intake, common in well-planned vegan diets, can mask the blood-count picture of B12 deficiency while neurological risk continues, which is one more reason to test B12 directly rather than infer it from feeling fine. Metformin and acid suppressants lower absorption of food-bound B12.

Run it in four steps. Three, pick a routine you will actually keep, fortified foods daily or a supplement, and note the started date so any future test is interpretable. Common misconceptions

That a varied vegan diet covers B12: unfortified plant foods do not, and feeling fine reflects liver stores, not intake. That nutritional yeast is inherently a B12 source: only when fortified, and the label says which. That testing can wait until symptoms appear: the neurological features are the ones worth never meeting, and the test is cheap. That a normal total B12 settles it for a long-term vegan: the indeterminate zone and the active test exist precisely because the total is a blunt instrument.
