Low-FODMAP diet under specialist dietetic input is second-line. This plan provides L-glutamine 2.4 g/day (Zhou 2019 RCT, doi 10.1136/gutjnl-2017-315136, used 15 g/day in post-infectious IBS-D with intestinal hyperpermeability; this plan dose is below the trial dose), psyllium husk 1.4 g (NICE-aligned soluble fibre), and a 7-strain Lactobacillus/Bifidobacterium probiotic warranting a 4-week trial per NICE. Inulin 1.2 g per scoop is a fermentable FODMAP and may aggravate symptoms in some patients; start low. For alarm features (rectal bleeding, weight loss, persistent symptoms despite dietary management), GP-led management is appropriate.
