Fermented dairy products

As mentioned above a lack in Lactobacillus bacterial strains can be linked to recurrent UTIs. Try to incorporate 1-2 tablespoons or organic, unpasteurised kefir (goat, sheep or cow based) or unflavoured yoghurt most days to support this. 

Hydration

Regular flushing of fluid through the system helps to keep it clear. For most people an intake of 1.5-2.5 litres of still water, with added unflavoured electrolytes, is sufficient. Avoid squash (including sugar free), juices, and cordials totally. Herbal teas containing dandelion and nettle are great for the urinary system.

Be aware of the impact of caffeine

Coffee, black and green teas are all diuretics which means they increase the need to urinate. Where someone is symptomatic this can cause increased discomfort so consider swapping away from these to allow the system to recover.

Per NICE NG112 (Urinary tract infection, recurrent: antimicrobial prescribing, 2018, last updated December 2024), first-line is behavioural and personal hygiene measures (adequate hydration, voiding after intercourse, wiping front to back). Women not pregnant with recurrent UTI may wish to try D-mannose (Kranjcec 2014 RCT, n=308, 2 g per day reduced recurrence 14.6 percent versus 60.8 percent no-treatment comparator over 6 months, NNT 3, high-quality evidence per NICE) or cranberry products (NICE-acknowledged but benefit uncertain). Standard antibiotic prophylaxis options include nitrofurantoin, trimethoprim, or methenamine hippurate (new 2024 NICE recommendation); topical vaginal oestrogen is recommended for postmenopausal women. This plan provides D-mannose 243 mg per cap and cranberry extract 99 mg per cap in Urinary Tract Support, plus Lactobacillus rhamnosus LMG 25626 at 10 Bn CFU per g (NICE notes some lactobacillus strain evidence but overall inconclusive). For red-flag features (fever, flank pain, nausea, men with UTI, pregnancy with UTI, visible haematuria, symptoms beyond 48 hours), urgent GP review per NICE NG109 is appropriate.
