Adenosylcobalamin is the mitochondrial form of B12, serving as a cofactor for the enzyme methylmalonyl CoA mutase within the mitochondria. This enzyme is essential for energy production from odd chain fatty acids and branched chain amino acids. When adenosylcobalamin is insufficient, methylmalonic acid accumulates, which can be measured in blood or urine as a specific marker of functional B12 deficiency.
